In case treatment with eye drops and laser for glaucoma failed, we can do eye surgery for glaucoma which will create a new opening to drain the aqueous fluid and so decrease the I.O.P. This kind of surgery is called filtering surgery
Peripheral laser iridectomy
In this procedure, the doctor will create a hole in the peripheral part of the iris either surgically or by using laser. This hole will communicate between the posterior chamber and the anterior chamber. This procedure can be done as a prophylactic procedure for patient who is at risk of closed angle glaucoma or can be part of the treatment plan of patient with closed angle and the I.O.P does not come down with eye drops.

Goniotomy
In this surgery, the doctor will create a hole in the trabecular meshwork that will help to drain the fluid. This kind of procedure is usually done for infant and children with high I.O.P.
Trabeculectomy
This type of surgery is one is the most common eye surgery for glaucoma these days.
In this procedure the doctor will create a new hole in the sclera near the cornea. This hole will connect inside the eye or anterior chamber to outside the eye. The outer part of the hole is covered by the conjunctiva. This hole will allow the aqueous do be directed outside the eye and so help to decrease the I.O.P.This procedure is about 80 % effective in lowering I.O.P
As in all surgeries, this procedure has many complications like infections, inflammation, over drainage of fluid, fibrosis or scar of the hole and decrease in the amount of fluid drained, cataract, retinal and Choroidal detachment.
With the use of antimetabolites drug intra-operatively, the risk of fibrosis or scar will decrease. The most common antimetabolites used are 5-fluorouracil and mitomycin C.
To decrease the risk of these complications, you should have regular follow up and you should continue using the eye drops that your doctor gave you. If you notice any unusual symptoms like loss of vision or eye pain, you visit your doctor as soon as you can

Drainage implants or shunts
Through this procedure, the doctor will insert a tube inside your eye through a hole in the sclera and near the cornea. This tube is connected to a plate which is implant on the sclera at the side of the eye and is covered by the conjunctiva. The tube will help to drain the fluid from the anterior chamber to the plate. The most common drainage implant is Ahmad valve
